Letter Templates
You will find templates for letters, including maternity leave and retirement, along with the form for a leave of absence. The templates will prompt you to make a copy, and you just need to insert the necessary information. You need to be signed into your gcsny account to access the templates. You will need to print and sign the letters/forms and submit them to the respective administrator. Please see your building rep for guidance.

You will also need to attach the Leave of Absence Form.
Fill out the form according to the leave needed.
